I believe its a hidden extra on the USA DVDs...

Berserk Vol. #2 Go to the extras menu, and while in it you press left, you'll light up a symbol on the left torch. Press enter to see a chibi picture of Nosferatu Zodd.

It looks about the right size for an avatar to me. They don't have to be 4000 pixels high to stand out, you know.
