The Behelit started to form a face, slowly, but it was forming. Then Slan said the main thing she could sense was fear. and then Guts used is dragon slayer to stab her eventually. But it was as if the behelit was going to activate?
Why did the beherit almost activate when Guts was confronted by Slan in the cave?

It didn't "almost activate", it just reacted to Slan's presence. As you know, a beherit's purpose is to take someone to the God Hand, which explains this reaction. Note that it was also rattled in volume 21 when Femto was incarnated, and that Ganishka's second rebirth as "Shiva" got it upset halfway across the world.
