As of now just how badly scarred is Guts? The only character that I can think of who has even more then Guts would be Shishio Makoto from Rurouni Kenshin.

In a real world scenario, the amount of scarring Guts has over his body would severly limit his mobility unless he went through a great deal of physiotherapy, and even then his flexibility and ability to do what he does (swing around a couple hundred pounds of iron like it was cardboard) would be... limited.
Basically, without the armor, misc magical healing and etc., in a real world case, with that much scarring, he'd have trouble walking.
