Anyways it's not that easy for the parents, yeah i know it's their fault, but imagine you denying giving food to your child, when she's always hungry.
I know someone like that, she's 4 years old and she already weights 40kg..
It's a psychological illness only a strong will can change that, not surgery..
Anyways that's just my 2 cents.
